I had exactly the same problem, where my printer has no borderless a5 setting and kept detecting the edge of the card leaving a 5mm blank strip. With a bit of effort, I finally got round it. As has already been said, set up your document as a4 and with your a5 bit you want to print landscape at the top. Stick the unfolded a6 card (a5 unfolded) to a sheet of a4 paper so both 210mm edges match up neatly. Use Magic tape for this as it's removable and will not damage your card. Then print as normal. I have an Epson sx515w with a top loading feeder so place the a4 paper with the card attached so that the card goes in first and faces the front. Also, I make sure when I set up my a4 document in Indesign, I extend the background colour past the a5 boundary by a couple of cm just to make sure it will print to the edge of the card. It works for me although takes time, hope that works for you.
