I have also had a few Canon chips go bad. It is a rare occurrence, but it seems Canon chips can go bad for no apparent reason. See this.
I think possible causes for chip malfunction are dirty chips, dirty or misaligned chip contact springs in the print head carriage, and a failing resetter. If your resetter is battery powered, maybe the battery/ies should be changed.
To avoid ink getting onto the chips and causing problems, it is a good idea to reset before you refill.
